Description:
Prequalifications are invited for Provision of Insurance Services (Health/Medical, Gpa/ Workmen Compensation, Inventory/Asset and Vehicle/Fleet Insurance) 1) Accessing the prequalification forms and details: Interested applicants shall obtain further information regarding solicitation documents and or address any request for clarification to: procurements_pumra@jsi.com 2) Deadline for request lor clarification and submission of applications: Any requests for clarifications should be made in writing by email only and sent to the following address: procurement_pumra@jsi.com and MUST be received no later than ten (10) days prior to the deadline for the submission of Applications The closing date for submitting a complete pre-qualification application is Monday 20th June not later than 04:00pm. 3) Labeling and marking of submissions: Submissions MUST be contained in a single large envelope clearly labeled "Application for prequalification as a Supplier/Service Provider (whichever is applicable)" and MUST indicate the category for which you are applying. 4) Address for submission of applications: Sealed applications should be addressed, and hand delivered to USAID PMI Uganda Malaria Reduction Activity - Lotis Towers. 16 Mackinnon Road, Kampala.
