Description:
Prequalifications are invited for Provision of media consultancy services. Eligibility Requirements: Entities expressing interest must demonstrate and submit the following compliance documents. 1. Type approval certificates from the Uganda Communications Commission (UCC) for radio communication and telecommunications equipment. 2. Electricity Regulatory Authority's {ERA) Company Installation Permit for electrical installations 3 Certified URSB Form 1(Notice of Beneficial Owners Particulars) 4. Certified copy of Annual returns with the registrar of companies for the last 3 years 5. Audited Financial Statement with the independent Auditor's opinions for the (last 3 years) 6. Governance Structures 7. Company or Business Profile 8. Certified Company Form 18 9. Certified Company Form 20 10. Certified certificate of incorporation or registration 11. Certified copy memorandum and articles of association 12. Certified statement of particulars for bidders participating under a business name, partnership. 13. Identification documents for persons or entities listed as director(s) and company secretary. partner(s) or proprietor(s) 14. Valid business licenses. Tax Registration Certificate, 15. Certified copy of a valid Trading License. 16. Valid NITA-U Certificate 17. Valid certificate of registration with the Personal Data Protection Office (PDPO) 18. Relevant experience of the bidder (The bidder should provide proof of implementation of at least five (5) (similar) projects in the past Seven (7) years. evidenced by contracts, reference letters, and state The amount of contract.) 19. Proof of experienced and certified key personnel to deploy and support the proposed services or works in respect of civil, mechanical, and electrical works. (Signed CVs and Certifications must be attached) 20. Filled out and Signed Supplier questioanre. 21. Signed Offer and Declaration Form 22. Tax Clearance Certificate, and 23. Other documents showing capacity to perform services or deliver goods.
